29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions had been taken to ensure their safety: (a) On or about June 16, 2014, for employees exposed to cave-in hazards while installing and patching a storm sewer approximately 8 feet deep without protective systems in place at or near the northwest corner of Exit 294 on Interstate 94 and ND State Highway 54 in Valley City, North Dakota.
