Employees did not travel from site to site during the day. They stayed onsite for the duration of the work shift. In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or they did not have smooth, well-rounded surfaces on which the cords may bear: At the project on or about November 14, 2013, there was no cover plate for a receptacle in a junction box that lay on the floor. The receptacle provided power to a 110 VAC Makita circular saw, model number 5007N.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws: At the project on or about November 14, 2013, there was no strain relief for an extension cord that entered an electrical box. The connector on the electrical box was designed to connect PVC electrical conduit to the box, could not be secured to the extension cord, and could not prevent pull on the receptacle terminal screws or joints. The box contained a 110 VAC electrical outlet that supplied power to a 110 VAC Makita circular saw, model number 5007N.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement: At the project on or about November 14, 2013, dozens of rebars were not guarded against impalement. The 3/8s inch diameter rebars protruded vertically about 36 to 40 inches from ground level. The rebars protruded from concrete masonry units on the perimeter of the property. Some rebars onsite were capped.
