29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: On the top level of the house, eye protection was not used while using a hammer to pound nails into lumber, exposing employees to the hazards of being struck by the nails.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not being protected by protective helmets: On the top level on the house, head protection was not worn while working near employees that swung around and carried sticks of lumber and rebar, exposing employees to the hazard of being struck by the lumber and rebar.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: On the top level of the house, there was no fall protection for employees that were building formwork and guardrails and moving rebar, exposing employees, along the 33 foot wide by 42 foot long perimeter, to the hazard of falling from heights that varied from about 8.66 feet to the roof of the garage to about 20 feet, 1 inch to the ground.
