29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: At the project site on November 18, 2013, a 50 foot extension cord was missing its ground pin. The cord powered a 110 VAC Black and Decker 7 ¼ inch worm drive saw, used to cut wooden forms.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. At the project site on November 18, 2013, a 50 foot extension cord was not protected from damage against vehicular and foot traffic. The cord powered a 110 VAC Black and Decker 7 ¼ inch worm drive saw, used to cut wooden forms.
