29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: On May 1, 2014, at the Guam Naval Base NEX Minimart and Gas Station Construction Project, a YANMAR Crawler Backhoe was operated with an unapproved bolt and washer on the arm of the heavy operating equipment and exposed employees to struck-by hazards.
