(a) Harmon Industrial Park, Driveway LSG Sky Chefs: On or about April 21, 2015, a lower safety guard of Makita 2416S Portable Cut-off saw was not secured in place with a proper wing bolt and knob to prevent applying excessive pressure on the handle, exposing employees to the hazards of being struck by the work piece to be ejected or struck by a breakage of the wheel.
