29 CFR 1910.179(j)(2)(iii): The employer did not conduct frequent inspections of hooks, and did not keep a certified record of monthly hoist hook inspections: (a) Springs Fabrication, Inc. at 3951 MacArthur Ave., Loveland, CO 80538: On or before November 18, 2013, the employer did not conduct frequent inspections of hooks involving, but not limited to, two 10-ton overhead cranes and a 2-ton overhead crane located in the welding/fabrication area of the North Shop, and did not keep a certified record of monthly hook inspections that includes the date of the inspection, the signature of the person conducting the inspection, and an identifier of the hook inspected. The condition exposed employees to the hazard of being struck by heavy loads.

29 CFR 1910.179(m)(1): Thorough monthly inspections of rope conditions, with written, dated and signed reports, were not performed on the running ropes: (a) Springs Fabrication, Inc. at 3951 MacArthur Ave., Loveland, CO 80538: On or before November 18, 2013, the employer did not conduct monthly inspections of rope conditions used with, but not limited to, two 10-ton overhead cranes and a 2-ton overhead crane located in the welding/fabrication area of the North Shop, and did not keep a certified record of monthly rope inspections that includes the date of the inspection, the signature of the person conducting the inspection, and an identifier of the rope inspected. The condition exposed employees to the hazards of struck-by and caught-in-between hazards as a result of a failure involving the running rope.
