OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to amputation and caught-in hazards while operating machining equipment without being adequately trained on safe machine operations: a) Machine Shop: On or about July 30, 2015 and at times prior, untrained, unskilled, and unsupervised employees were permitted to operate fabrication equipment including a HAAS SL-30 Computer Numerically Controlled (CNC) Lathe. Specifically, a Machinist Helper was not provided with training, a training program, or job hazard assessment prior to being assigned to operate the machine. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
