Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: a) On or about June 26, 2025, employees were exposed to "struck by" hazards from steel storage racks created by the base plates on storage rack columns were not anchored to the floor. Abatement Note: Feasible and acceptable means to correct this hazard involve having employers follow the practices set forth in ANSI MH16.1-2023: Industrial Steel Storage Racks, including but not limited to the following specific sections below: These standards, along with the storage rack manufacturers specifications, mandate that all rack columns be anchored to the floor with anchor bolts designed to resist applicable forces. At a minimum, one anchor per upright column is required, though two are often recommended to prevent pivoting.
