OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees while raising and positioning a load using a Close Proximity Operated Manually Controlled Permanent Magnet:  a) Hydro Shop: On or about April 1, 2022, workers were exposed to crushing injuries while using a magnetic lifting device rated at 250 pounds to lift and position a flange blank weighing 298 pounds onto a pressure vessel flange opening.  The magnet was not rated for the load in that the rated capacity was 250 pounds and the load, a flange blank, weighed  298 pounds.  A side pull occurred when the flange blank caught on a receiving lug.  This created a side pull force that disengaged the magnet from the flange blank, which then fell into position while the operator's finger was resting on the mating surface.
