29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the slope of the excavation was steeper than one and one half horizontal to one vertical and the operator did not use any other option provided for in 29 CFR 1926.652(b).      (a) Big Hollow Diversion Pump Station, Fox Hollow Road, State College Pennsylvania - An employee worked in a trench 28 feet long and 12 feet wide.  The south wall of the trench was cut vertically for at least 3 feet and then cut back on a slope.  The north wall of the trench was cut vertically seven feet down below a gas line and then benched for 3 feet horizontally and 3 feet vertically.  No other protective system was used.  The employee was exposed to possible trench collapse on or about November 19, 2013.         Tru-Line Contractors Incorporated was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314242306, citation number 1, item number 2 and was affirmed as a final order on December 14, 2010, with respect to a workplace located at 54 North Middlesex Road, Carlisle Pennsylvania.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
