29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a)  On or about November 25, 2013, employees were exposed to injuries from falls, while conducting reroofing activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface.  Note:  This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 315715367, Citation Number 1, Item Number 1, and was affirmed as a final order on October 14, 2011, with respect to a workplace located at 8101 Cantrell Road, Little Rock, AR, 72227.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  (a)  On or about November 25, 2013, the employer did not protect workers from being exposed to fall hazards while using portable ladders that did not extend at least 3 feet above upper landing surface being accessed.     All-Tex Roofing, LLC, was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.1053(b)(1) or its equivalent which was contained in OSHA inspection Number 315715367, Citation 1, Item 2, and was affirmed as a final order on October 14, 2011, with respect to a workplace located at 8101 Cantrell Road, Little Rock, AR, 72227.
