On or about 4/12/2021, employees from All-Tex Roofing LLC were repairing a roof of a residential building located at 8650 Kingsbridge Lane in St. Louis, MO. Two employees were installing roofing materials with pneumatic nail guns without wearing eye protection.

On or about 4/12/2021, employees from All-Tex Roofing LLC were repairing a roof of a residential building located at 8650 Kingsbridge Lane in St. Louis, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The foreman and four employees were observed working on the roof, approximately seventeen feet above the ground, without utilizing fall protection.

29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about 4/12/2021, employees from All-Tex Roofing LLC were repairing a roof of a residential building located at 8650 Kingsbridge Lane in St. Louis, MO. The foreman and four employees were using an extension ladder to access the roof. The ladder was not extended three feet beyond the landing or edge of the roof.
