29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard not attached to the boom or basket when working from an aerial lift: (a) Deerfield Academy Performing Arts Building: On or about November 4, 2013, 2 employees were riding in an aerial lift, and neither was attached to the boom or basket, which is not in accordance with this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.760(d)(2): Fall arrest system components used in fall restraint systems did not conform to the criteria in part 1926 section 502; specifically paragraph (d) subparagraph (6 v) which states that, 'snaphooks shall not be engaged to any object which is incompatably shaped or dimensioned in relation to the snaphook ...': (a) Deerfield Academy Performing Arts Building: On or about November 4, 2013, an employee was partially attached to the structural steel he was working on/from by his snaphook being clipped to a structural steel member that was much larger than the snaphook opening thus rendering the connection incomplete, which is not in accordance with this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
