29 CFR 1926.757(a)(9): Steel joists and steel joist girders were used as anchorage points for a fall arrest system without written approval to do so from a qualified person. Jobsite: Fifth floor, east side of building adjacent to interior courtyard. An employee tied off to steel joists that he was welding to an open web girder. No written approval from a qualified person had been obtained before tying off to the joist. The employee was exposed to a fall hazard of approximately 43 feet.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan. Jobsite: Stair landings, Stairs leading from ground to third floor. Stair landings on stairs used to access the building's upper floors lacked 'filler' materials to the top edge of each landing.
