29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18-inches wide:  (a) On or about 11/25/2013, an employee was observed standing on a wood platform at the third floor balcony of the construction of the SOHO Square Apartments in Tampa(FL). The width of the platform was only 10-12 inches wide exposing the employee to a 37-foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.    (a) for the employee working from a wooden platform at the third floor balcony of the construction of the SOHO Square Apartments in Tampa (FL), as observed on or about 11/25/2013.  No safe access had been provided which required the employee to climb the wooden supports to access and egress the working level.  This exposed the employee to a 37-foot fall hazard.

(a) for the employee working from the third story balcony at the SOHO Square Apartments in Tampa (FL), as observed on or about 11/25/2013.  The employee was observed working from a wooden platform/scaffold and exposed to a fall hazard of over 37-feet.  The scaffold had not been erected by a competent person.

The leg straps of the safety harness were not being used by the employee.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  (a) for the employee working at the third floor balcony of the construction of the SOHO Square Apartments in Tampa FL), as observed on or about 11/25/2013.  The employee was using a safety harness and safety line but a safe anchorage point had not been properly selected or used; the safety line was simply tied to a wooden member of the structure exposing the employee to a 37-foot fall hazard.
