29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Employees in the process of roofing activities, on a 5.5 in 12 slope (vertical to horizontal) roof, were exposed to a 20 foot fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 12/18/2013.  FLG Construction Inc., previously known as JAD Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315713867, citation number 1, item number 1 and was affirmed as a final order on 08/18/2011, with respect to a workplace located at 2121 Valterra Vista Way in Valrico, Florida.  FLG Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 916340, citation number 1, item number 1 and was affirmed as a final order on 08/09/2013, with respect to a workplace located at 14322 Avon Farms Dr., Tampa, Florida.  FLG Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 940574, citation number 1, item number 1 and was affirmed as a final order on 11/22/2013, with respect to a workplace located at 7107 Peregrina Loop, Wesley Chapel, Florida.
