Oxygen and acetylene cylinders used for cutting torches were stored directly adjacent to the load extension backrest of a forklift and subject to being damaged. 29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  a) Maintenance Area. 29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure     a) Room #3.  A flexible extension cord (110v) was plugged into a wall outlet and strung along the wall along two sides of the room and down to the floor area where it was plugged into another extension cord that was plugged directly into a portable conveyor in order to power the conveyor.      There is no abatement documentation or certification required for this item.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  a)  Room #3 Blenders.  The mesh screen guard covering the 5" x 5" grate openings to the blender blades were neither affixed to the machine nor interlocked, potentially exposing employees to the mixing blades if the mesh screens were not in place.
