Filler guard door interlocks were not working either.     d)    On or about 12/13/2017, Line 3B was being operated without interlocks on the guard doors to prevent operator from reaching into the danger zone during the operating cycle.  Amputation occurred when employee opened guard door to remove pouch that was stuck on a clip without shutting off machine first.

Subco Foods, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1108197, citation number 2, item number 1 and was affirmed as a final order on February 29, 2016, with respect to a workplace at 4350 South Taylor Drive, Sheboygan, WI 53081.
