29 CFR 1910.28(c)(1): (1) Toe boards, screens, or guardrail systems were not erected to prevent objects from falling to a lower level;    On or about 8/30/2017,  the platform for mixer #10 & mixer #11 have an 11 inch gap between the mixer frame and the platform deck in several areas around the equipment that do not have the protection of a toe board. The hand rail on the platform also had one section with a 4 inch gap between the toe board and grating.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a)    On or about 9/20/2017, Paxall cartoner for line 7A did not have interlocks functioning for guard doors to prevent operator from reaching into the danger zone during the operating cycle.    b)    On or about 9/20/2017, Bartelt machine for line 7A did not have guard doors secured by special tools as designed for closure by the manufacturer.    c)    On or about 9/20/2017, Line #25 had 3 guard doors missing (2 on west side of machine and 1 on east side of capper).  Filler guard door interlocks were not working either.     d)    On or about 12/13/2017, Line 3B was being operated without interlocks on the guard doors to prevent operator from reaching into the danger zone during the operating cycle.  Amputation occurred when employee opened guard door to remove pouch that was stuck on a clip without shutting off machine first.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a)    On or about 9/20/2017, Flexible extension cord was run through the wall outside of the Packaging Office for the AMANO time clock that all 42 employees used twice daily.    b)    On or about 9/20/2017, Flexible extension cord was run through west wall of plant to an outside  shed to provide power in building.    Subco Foods, Inc. was  previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 950981,  citation number 1, item number 2 and was affirmed as a final order on June 10, 2014, with respect to a workplace located at 1150 Commerce Drive, West Chicago, IL 60185.    Subco Foods, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1108197, citation number 2, item number 1 and was affirmed as a final order on February 29, 2016, with respect to a workplace at 4350 South Taylor Drive, Sheboygan, WI 53081.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.    On or about 12/13/2017, Employee reached into a machine without turning it off first and received a right middle distal phalanx traumatic amputation while operating Bay 3B line. OSHA was made aware of the injury on 1/17/2018 while making a site visit for an ongoing inspection from 9/20/2017.
