29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. On or about December 16, 2013, at a jobsite, employees were not instructed to recognize and avoid unsafe conditions to control or eliminate the hazards of a wall collapse. Inadequate consideration of such conditions are not limited to the following: (1) raising a 12 feet 1 inch high by 26 feet wide wall; (2) raising a wall without mechanical aids, such as wall jacks, crane, or an adequate forklift; (3) raising a wall that weighs approximately 882 pounds and; (4) raising a wall without the use of blocking or cribbing to prevent the crushing of employees in case of failure.
