29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. On or about May 6, 2014, a Werner 12-foot extension ladder used for access and egress from the roof area was not extended 3 feet above the landing, exposing the employees to a 9 feet fall hazard.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about May 6, 2014, the employee using a DeWalt and Makita circular saws were not wearing safety glasses, exposing himself to eye injuries.  The Fastway Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 951659, citation number 01, item number 002 and was affirmed as a final order on February 27, 2014, with respect to a workplace located at Sheepshank Lane, located in Santa Rosa Beach, FL.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about May 6, 2014, roof area, two employees installing hardy board siding was not protected from falling 9 feet by the use of a guardrail system, safety net system, personal fall arrest system or any other type fall protection method.  The Fastway Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 951659, citation number 01, item number 002 and was affirmed as a final order on February 27, 2014, with respect to a workplace located at Sheepshank Lane, located in Santa Rosa Beach, FL.
