29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    On or about December 19, 2013 and times prior thereto located at 199 Extrusion Place in Hot Springs, Arkansas, employees were not provided with personal protective equipment while operating a forklift without a functioning seatbelt.  The seat belt was not in working condition.  The employees were exposed to the hazard of being ejected and crushed in the event of a tip-over or accident.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    On or about December 19, 2013 and times prior thereto, located at 199 Extrusion Place in Hot Springs, Arkansas, an employee was allowed to operate a forklift without successfully completing training and evaluation to ensure his competence.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc:    On or about December 19, 2013 and times prior thereto, located at 199 Extrusion Place, employees operating the Wysong Press Brake #HP B62-100 was exposed to the hazards associated with the point of operation.  Employees were forming small parts and no guarding was in use to protect employees from crushing hazards.

29 CFR 1910.252(b)(2)(iii): Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles:      On or about December 19, 2013 and times prior thereto, located at 199 Extrusion Place in Hot Springs, AR, employees adjacent to the welding area were observed being exposed to rays without the use of screens, shields, or appropriate goggles.

29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment shall contain an equipment grounding conductor:    On or about December 19, 2013 and times prior thereto, employees were exposed to the hazard of electrical shock while using a Goldstar microwave (SN# A90305427) on the production floor that had a missing ground plug on its power cord.    The Alum-weld, Inc. dba Xpress Boats was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334 (a)(3)(i), which was contained in OSHA inspection number 770041, citation number 1, item number 4 and was affirmed as a final order on February 14, 2013, with respect to a workplace located at 199 Extrusion Place, Hot Springs, AR  71901.
