29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On September 12, 2017 and times prior thereto, at the worksite located at or near 199 Extrusion Place in Hot Springs, Arkansas, the light sensors provided for the aluminum baler, labeled 4200 with serial number 5905, were not effective due to employees being able to open the door of the baler while running. This condition exposed employees to amputation hazards.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles, to protect them from the rays when noncombustible or flameproof screens or shields were not used: On or about September 14, 2017 and times prior thereto, at the worksite located at or near 199 Extrusion Place in Hot Springs, Arkansas, the employer did not have any eye protection or around employees welding. This condition exposed other employees in the vicinity to eye injuries. The Aluma-Weld Inc. dba Xpress Boats was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.252(b)(2)(iii), which was contained in OSHA inspection number 952826, citation number 1, item number 4 and was affirmed as a final order on April 04, 2014 with respect to a workplace located at 199 Extrusion Place.
