OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a) DH Steel Products, LLC - on or about January 23, 2014, employees were potentially exposed to struck-by hazards from potentially defective hoisting equipment due to the lack of an inspection program.  Hoists including, but not limited to, the Harrington Hoist, were not incorporated into an inspection program.  The hoists were utilized for activities including, but not limited to, lifting and transferring steel beams overhead hoists used to lift and move were not subjected to frequent and periodic inspections.

Servicing and maintenance is conducted on machinery that included, but was not limited to, the ironworker, press brake and shears.

Potential obstructions projected into the aisles that included but were not limited to fabrication stands, beams, fabricated pieces, and pipes.
