Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On 2/21/2014, a siding installer from Warren Exteriors was installing siding while working on the roof at a residential building located at 1343 Shorewinds Trail, St. Charles, MO.  He was not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employee was observed on the roof of the building without fall protection, exposing him to fall to the ground below.

On 2/21/2014, a siding installer from Warren Exteriors was installing siding while working on the roof at a residential building located at 1343 Shorewinds Trail, St. Charles, MO.  He was using a ladder improperly to access the roof.  The ladder was not extended three feet beyond the landing or edge of the roof.
