29 CFR 1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold was released for use by employees other than erection/dismantling crews.  On 12/30/2019, employees of Warren Exteriors LLC were installing siding on a residential building located at 234 Mason Glen Drive, Lake Saint Louis, MO.  The Pump Jack Scaffold employees were working from did not have guardrail systems installed along all open sides and ends of platforms.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On 12/30/2019, employees of Warren Exteriors LLC were installing siding on a residential building located at 234 Mason Glen Drive, Lake Saint Louis, MO.    The employer allowed employees to work on scaffolds without adequate training by a person qualified in the subject matter.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.  On 12/30/2019, employees of Warren Exteriors LLC were installing siding on a residential building located at 234 Mason Glen Drive, Lake Saint Louis, MO.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.

29 CFR 1926.501(b)(13):  "Residential construction." Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 12/30/2019, employees of Warren Exteriors LLC were installing siding on a residential building located at 234 Mason Glen Drive, Lake Saint Louis, MO.  Employees were installing siding while standing on the roof approximately 11 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection.  Warren Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1273795, citation number 1, item number 2 and was affirmed as a final order on 11/29/2017, with respect to a workplace located at Glen Forest Drive, Troy, Missouri.
