29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying and correcting existing hazards):  At the Glen Forest, Troy, MO construction site, where the employer was installing siding, the competent person was unable to or did not determine how and where to install personal fall arrest system anchors in order to prevent an employee from contact with the ground, when working from the roof or ladder jack scaffolds.  Abatement documentation is required for this violation.

Abatement documentation is required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   At the Glen Forest, Troy, MO construction site, siding work, while working on the roof, was performed without fall protection, such as wearing a harness properly tied off to an anchor.
