29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) North end of townhouse located at 380 Swingline Lane, Milton, Georgia. The upper most working platform of the fabricated frame scaffolding system was not fully planked or decked exposing employees to fall hazards of approximately 19-feet 7-inches.

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: a) North end of townhouse located at 380 Swingline Lane, Milton, Georgia. Employees were not protected from falling from the fabricated frame scaffold working platforms by the use of a guardrail or safety net system or personal fall arrest systems. Two employees were exposed from the scaffold platform 19-feet 7-inches above ground. The platform did not have a complete guardrail system in place, the mid-rail was missing. b) North end of townhouse located at 380 Swingline Lane, Milton, Georgia. An employee was not protected from falling from the fabricated frame scaffold working platforms by the use of a guardrail or safety net system or personal fall arrest systems. The employee was exposed from the scaffold outrigger 17-feet 8-inches above ground level. The end of the outrigger did not protect from fall hazards.
