29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:    On or about April 28, 2015, at 7650 Settles Walk Lane, Suwanee, Georgia:    Two employees performing brick masonry work were exposed to a fall hazard of approximately 18-feet and greater when they worked from the work platform of a tubular welded frame scaffold that did not have toe boards, guard rails, a ladder fully installed and whose scaffold poles were set on top of scaffold planks laid on top of an approximately 4/12 slope porch roof.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.     On or about April 28, 2015, at 7650 Settles Walk Lane, Suwanee, Georgia:     Two employees performing brick masonry work were exposed to fall hazards of up to approximately 18-feet and greater when the employer did not install a ladder all the way to the top work platform.  To access the third level, the employees had to climb the scaffold frame from the second level.    AGP Brick, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 954133, citation number 1, item number 3, and was affirmed as a final order on May 7, 2014, with respect to a workplace located at 7605 Harbison Hollow Lane, Suwanee, Georgia.

AGP Brick, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(4)(i), which was contained in OSHA inspection number 954133, citation number 1, item number 5, and was affirmed as a final order on May 7, 2014, with respect to a workplace located at 7605 Harbison Hollow Lane, Suwanee, Georgia.
