29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.     On or about March 7, 2014, at a residential structure located at 14201 Overcreek, Woodland's Edge, Little Rock, Arkansas 72211, an employee was bricklaying the back of residential structure from scaffold. The employer did not ensure platforms on all working levels of scaffolds were fully planked. Employee was observed/exposed to trip and elevated fall hazards of greater than 14 feet to unfinished ground below from scaffold.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On or about March 7, 2014, at a residential structure located at 14201 Overcreek, Woodland's Edge, Little Rock, Arkansas 72211, an employee was bricklaying the back of residential structure from scaffold.  The employer did not ensure an employee was properly protected by the use of fall protection systems while working on scaffold. An employee was observed/exposed to fall hazards of greater than 14 feet to unfinished ground below from scaffold.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement.    On or about March 7, 2014, at a residential structure located at 14201 Overcreek, Woodland's Edge, Little Rock, Arkansas 72211, an employee was bricklaying the back of residential structure from scaffold.  The employer allowed employees to work on scaffolds with unstable footings, where unstable pieces of lumber and brick were stacked to support the scaffold. Employees were observed/exposed to scaffold collapse and elevated fall hazards of greater than 14 feet to unfinished ground below.

On or about March 7, 2014, at a residential structure located at 14201 Overcreek, Woodland's Edge, Little Rock, Arkansas 72211, an employee was bricklaying the back of residential structure from scaffold. The employer did not ensure crossbracing was installed on both sides of scaffold. The employee was observed/exposed to scaffold collapse and elevated fall hazards of greater than 14 feet to unfinished ground below from scaffold.

The employer did not ensure an employee was properly trained to recognize and avoid hazards when it was obvious they were exposed to fall hazards from improper scaffolding.
