29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set.    On or about March, 11, 2014 load engaging means were not lowered as the machine was left unatended, exposing an employee to a fall and or struck-by hazard.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.     On March, 11, 2014 an employee was not provided with a safe means of access to a ladder jack scaffold, exposing the employee to a fall of 14' 2".     Kos Remodeling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(e)(1) which was contained in OSHA inspection number 312726409, citation number 02 item number 01  and was affirmed as a final order on July 20, 2009, with respect to a workplace located at  422 E. 9 th St. Hinsdale, Illinois.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
