Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and crush hazards when semi-trailers positioned on ice slid away from the loading dock while forklift operators loaded and unloaded them.    a) Camaco LLC, Lorain, Ohio:  On or about February 26, 2014, the employer did not ensure that semi-trailers being loaded by lift trucks in the shipping and receiving department were secured to prevent movement during adverse winter weather conditions.  Multiple trailers had slid away from the docks during forklift loading/unloading activities after ice pockets had built up under the wheels and landing gear, exposing employees to crushing hazards, in the event of a forklift tip-over due to falling from the loading dock or trailer.    Among other methods, one feasible and acceptable method to correct this hazard includes, but is not limited to conforming to the American National Standard Institute/Industrial Truck Standards Development Foundation - ANSI/ITSDF B56.1-2012 - Safety Standard for Low Lift and High Lift Trucks, Chapter 4.14.1, by providing a means to prevent unintentional movement of highway trucks and trailers, such as repairing the current trailer dock lock system to hold the trailers in position during loading and unloading activities.

29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident:    a)  Camaco LLC, Lorain, Ohio:  On or about February 18, 2014, the employer did not ensure that a lift truck operator who had been involved in a near-miss incident received refresher training.  The operator had been unloading a semi-trailer when the trailer slid on ice away from the shipping/receiving dock. The lift truck nearly fell off of the back of said trailer, exposing the employee to crushing hazards.    b) Camaco LLC, Lorain, Ohio:  On or about February 26, 2014, the employer did not ensure that a lift truck operator who had been involved in a near-miss incident received refresher training.  The operator had been unloading a semi-trailer when the trailer slid on ice away from the shipping/receiving dock.  The lift truck had barely made it back onto the dock when the dock plate dropped down, exposing the employee to crushing hazards.
