Dockboard or bridgeplates were not driven over carefully and slowly and their rated capacity never exceeded. On 3/18/2014, an employee from Rural King Supply located at 15190 Veterans Parkway, Wentzville, MO, was unloading a trailer positioned against the dock. There was not a bridgeplate or dockboard available; therefore, the operator drove a Reallift Forklift, SN 100206751, over a seven inch gap between the trailer and loading dock to unload the trailer.
