29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a.)    Houser Products, LLC located at 173 Oak St. Marion, OH 43302; On or about March 18, 2014, the employer did not develop or document an energy control program consisting of employee training, periodic inspections, and  procedures.  Employees were exposed to amputation and caught by hazards when setting up and changing tools on machines such as, but not limited to, the VM20 CNC Mill (Serial #8392) and the Mazak Lathe (Serial #186583).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a.)    Houser Products, LLC located at 173 Oak St. Marion, OH 43302; On or about March 18, 2014, the employer did not assure employees were protected from injuries associated with rotating parts and flying chips while operating CNC machines such as, but not limited to, the Mazak Nexus 200MY CNC Lathe (Serial #186583) and the VM20 CNC Mill (Serial #8392). The machines were not adequately guarded in that employees were not prevented from accessing the hazard area during machine operation.

Employees that operated the Eagle Tube Bender to bend metal parts were exposed to amputation injuries in that the employer did not enure pinch points were guarded.    b.)    Houser Products, LLC located at 173 Oak St. Marion, OH 43302; On or about March 18, 2014, the employer did not assure employees were protected while operating the BLM Tube Bender (Serial #1540440100052). Employees that operated the Eagle Tube Bender to bend metal parts were exposed to amputation injuries in that the employer did not ensure pinch points were adequately guarded.

Houser Products, LLC located at 173 Oak St. Marion, OH 43302; On or about March 18, 2014, the employer did not meet the basic requirements for recording injuries and illnesses.  OSHA Forms 300, 300A, and 301 were not maintained for calendar year 2014. On or about March 6, 2014, an employee sustained a recordable injury that required medical treatment.
