Houser Products, LLC - Marion, Ohio: On or about June 1, 2015, the employer did not ensure the point of operation on US Industrial Machinery press brake (model US446R; s/n W2008-242) was adequately guarded to protect employees from amputation injury. Houser Products, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA inspection number 963824, citation number 1, item number 2 and was affirmed as a final order on July 17, 2014, with respect to a workplace located at 173 Oak Street, Marion, Ohio 43302.

Houser Products, LLC - Marion, Ohio: On or about July 13, 2015, the employer failed to report an employee amputation injury. On June 1, 2015, an employee sustained a finger-tip amputation while operating a press brake; the employer did not report the amputation within 24 hours.
