29 CFR 1910.151(c):   Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  (a) An eyewash was not immediately available in Room 204 (basement of hospital) where a custodian handled caustic bleach during the filling of spray bottles and while dispensing the chemical into a washing machine.

(a) The employer did not inform a housekeeping contractor of the presence and location of asbestos-containing floor tile in the hospital to which the contractor's employees were potentially exposed to while performing floor maintenance activities (e.g., buffing, stripping finish).  ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.1001(j)(5)(i): The building owner or employer did not affix or post labels or signs to notify employees of what materials contain ACM and/or PACM for previously installed ACM and/or PACM:  (a)  The employer did not affix or post labels or signs to notify employees of the presence of asbestos-containing floor tile in the facility.   NOTE: Since it is generally not feasible to put labels on floors, alternatives may be used. If asbestos-containing floors are being serviced by employees using a common equipment room day after day, then a sign or label for the asbestos flooring can be posted in the equipment room. The object is to forewarn employees who may be potentially exposed during the floor cleaning operation and have access to the material.

29 CFR 1910.1200(f)(6)(ii):   Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provided at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemicals:  (a) A 4-gallon metal container of gasoline found in the paint shop was not provided with identity or hazard warning labels.  ABATEMENT CERTIFICATION IS REQUIRED
