29 CFR 1910.23(e)(9): Wall opening barriers (rails, rollers, picket fences, and half doors) was of such construction and mounting that, when in place at the opening, the barrier was not capable of withstanding a load of at least 200 pounds applied in any direction (except upward) at any point on the top rail or corresponding member.   a) Center of the 2nd floor of the Power Plant: A barrier placed in front of the 64-inch by 62-inch  wall opening cut into the side of the Number-3 boiler was not secured to prevent it from falling. The barrier consisted of orange construction fencing which did not go the full width of the opening and a large piece of plywood which was simply leaned against the structure.   ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   a) East wall of the Maintenance Shop in Building 158 (Base Laundry): A Dayton bench grinder Model Number 42123G had the work rest on the left hand out 1/2 inch from the abrasive wheel, and the work rest on the right hand side was out 3/4 inch from the abrasive wheel.   ABATEMENT CERTIFICATION IS REQURED

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) East wall of the Maintenance Shop in Building 158 (Base Laundry): A Dayton bench grinder, Model Number 42123G, had a 1/2 inch space between the right hand side abrasive wheel and the tongue guard.   b) Center of the Machine Shop at Public Works: A Dayton bench grinder had a 3/4 inch space between the right and side abrasive wheel and the tongue guard. 29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  a) South wall of the Maintenance Shop in Building 158, Base Laundry: A Dayton pedestal fan was missing the ground prong from the plug-end.   b) East wall of the HVAC break area, Public Works Building: A Westinghouse refrigerator did not have a ground prong on the plug-end of the cord.   c) East wall near the HVAC Supervisors Office: A Whirlpool refrigerator did not have a ground prong on the plug-end of the cord.   d) South wall 2nd floor of the Power Plant: A TPI Industrial pedestal fan was missing the ground prong from the plug-end.   e) South end of Bay Number 4, Base Fire Department:  Dayton pedestal fan was missing the ground prong from the plug-end.
