29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: (a) Quality Welding & Fabrication, LLC on or about March 28, 2014, at the address of 2300 Elida Rd. Lima, OH: The employer failed to ensure an employee task with steel erection activities, was protected from impalement hazards while working from a ladder directly above inadequately guarded reinforcing steel. The reinforcing steel protruding approximately 3 ft. from a block wall, maintained a plastic abrasion cap providing insufficient protection from impalement hazards in the event of a fall.
