29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Quality Welding & Fabrication, LLC. on or about July 6, 2016, at the address of 4330 East Rd. Elida, OH: The employer did not ensure the point of operation on the Niagara HD-100-8-10 hydraulic press brake was adequately guarded. Employees operating the aforementioned machine were exposed to amputation hazards at the point of operation during hands-on stock production.
