29 CFR 1910.212(a)(3)(ii):  Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:    An employee working to form parts on the #2 AccuraPress was exposed to amputation hazards because the employee was not protected from contact with the die and the point of operation was not guarded to prevent employees from reaching into the die during an operating cycle.     Interlake Mecalux, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 92311, citation number 1, item number 3 and was affirmed as a final order on January 3, 2012, with respect to a workplace located at 701 Interlake Drive, Pontiac, IL 61764.
