29 CFR 1926.852(f): Where the material is dumped from mechanical equipment or wheelbarrows, a securely attached toeboard or bumper, not less than 4 inches thick and 6 inches high, shall be provided at each chute opening. a. On the top of the roof where employees wheel barrelled materials up to the chute openings, a bumper was not installed at the front of the chute to prevent employees and equipment from falling into the chute.

29 CFR 1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall: 29 CFR 1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level; a. On the West side of the roof where employees were applying rubber roof covering, the employee wearing a personel fall arrest system was rigged that in the event of a fall the employees would free fall 14 1/2 feet.

On the north side of the building where roofing work was being performed, there was a section measuring 10' 8" which did not have fall protection such as, but not limited to, a warning line thereby exposing employees to a 24 foot fall hazard.

29 CFR 1926.502(f)(1)(i): When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge. a. On the southwest side of the rear of the building, the warning line was installed at 4 feet from the edge of the roof thereby exposing employees to a 24 foot fall hazard. b. On the southeast side of the rear of the building, the warning line was installed at 5 feet from the edge of the roof thereby exposing employees to a 24 foot fall hazard. c. On the north side of the building, the warning lines were blowing over the front of the building thereby exposing employees to a 24 foot fall hazard. The Kalkreuth Roofing and Sheet Metal Company was previously cited for a violation of this occupational safety and health standard 1926.502(f)(1)(I), which was contained in OSHA inspection number 243010, citation number 1, item number 2 (a) and was affirmed as a final order on April 11, 2012, with respect to a workplace located at 4211 East Broad Street, Columbus, Ohio 43213.
