29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about October 21, 2014, during the installation of roofing material on a low slope or flat roof, the employer did not ensure employees were protected from a fall of approximately 14.3 feet by utilizing a guardrail system, fall protection system, and/or combination of warning line system and safety monitor system. KALKREUTH ROOFING AND SHEET METAL, INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(B)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 982076 , CITATION NUMBER 02, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 29, 2014, WITH RESPECT TO A WORK PLACE LOCATED AT 4209 STATE ROUTE 44, ROOTSTOWN, OHIO.

Warning lines protecting the Northwest corner of the building work area were lying down on the roof. KALKREUTH ROOFING AND SHEET METAL, INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.502(F)(1)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 243010 , CITATION NUMBER 01, ITEM NUMBER 002A, AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 11, 2012, WITH RESPECT TO A WORK PLACE LOCATED AT 4211 EAST BROAD STREET, COLUMBUS, OHIO.
