29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    a) Residential jobsite: On April 7, 2014, and at times prior thereto employees performed roofing activities on a 10 in 12 pitched roof without a fall protection system in place.  Employees were exposed to fall hazards ranging from 12 feet to 24 feet.       Note) Abatement certification IS required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:    a) Residential jobsite: On April 7, 2014 and at times prior thereto employees did not recognize or acknowledge the hazard of performing roofing activities on a 10 in 12 pitched roof without a fall protection system in place.  Appropriate actions to minimize the hazard were not taken.         Note) Abatement certification IS required for this item.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a) Residential jobsite: On April 7, 2014 and at times prior thereto housekeeping was not maintained in the areas where employees performed roofing activities, and in the area used for access to the roof.  Construction debris including but not limited to, scrap pieces of shingles and material wrapping paper were allowed to accumulate.  Note) Abatement certification IS required for this item.
