29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a)     On the roof: An employee was performing roofing activities on a 4:12 to 8:12 residential roof without fall protection systems in place. Employee was exposed to falls ranging from 10 feet to 18.4 feet.  Note: Abatement certification IS required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:    a)      On the roof:  the employer did not ensure employee understanding and comprehension of fall protection training  by employees engaged in residential roofing activities. One employee  had the rope grab of his life line at the knotted end after it draped over the eve, touched the ground and returned back to his location on the roof and attached to his lanyard.   Note: Abatement certification IS required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)     At the roof:  employees used an aluminum extension that extended  one rung (approximately one foot) above the eve of the roof. Employee was exposed to falls 9.3 feet.  Note: Abatement certification IS required for this item.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support:    a)       At the roof:  employees used an aluminum extension ladder that was at an angle of 49 degrees instead of the 75.5 degree angle to access the roof.
