29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501:    a) ) Residential jobsite: On April 10, 2014, and at times prior thereto employee(s) performing roofing activities on a 10 in 12 pitched roof were not properly attached to a fall protection system.  Employees were exposed to fall hazards ranging from 12 and a half feet to 24 and a half feet.      b)  Residential jobsite: On April 10, 2014, and at times prior thereto employee(s) performing roofing activities on a 10 in 12 pitched roof were not attached to a fall protection system.  Employees were exposed to fall hazards ranging from 12 and a half feet to 24 and a half feet.       Note) Abatement certification IS required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:     a) Residential jobsite: On April 10, 2014 and at times prior thereto employees did not recognize or acknowledge the hazards of performing roofing activities without being attached to a fall protection system, and not wearing fall protection properly.  Appropriate actions were not taken to minimize these hazards.   Note) Abatement certification IS required for this item.
