29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  (a) Press number 1 - On or about May 22, 2014  employee exposed to continuous noise at 97.2% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 89.8 dBA during the 425 minute sampling period of 05/22/2014. Exposure calculation included a zero increment for the time not sampled.
