29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: On or about 5/8/2014 Location: 3rd floor, 1191 Boston Road, Bronx, NY 10456 a) Three of the four wheel locks on a Werner SRS-72, 6 foot "Baker" type mobil scaffold were broken and missing. The scaffold was used by employees to set block walls.

29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: On or about 5/8/14 Location: 1191 Boston Road, Bronx, NY 10456 a) Numerous floor holes throughout the building that were bored through the concrete slab floor for piping and raceways and ranged in diameter from 3 to 6 inches, were covered by pieces of plywood that were not secured to the floor to prevent displacement.
