29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  Manufacturing Facility, Director Dr.: On or about May 21, 2014, and times prior there to, employee(s) operating an Exacta-Cut Package Saw (Model 116-12-104TMH) were exposed to continuous noise at a dose of 109.8% (8-Hour Time Weighted Average sound level of 85 dBA) during the 448 minute sampling period. For a full eight hour work shift this exposure is equivalent to an average sound level of 90.6 dBA. Exposure calculations include a zero increment for the 32 minutes not sampled.

29 CFR 1910.1200(g)(8): The employer did not ensure that material safety data sheets were readily accessible to the employees in their work area during each work shift:  a) Production Facility, Space Center Dr.: On or about May 20, 2014, employee(s) who utilized AW 46 6K Premium Anti-Wear Hydraulic Oil for the service/maintenance of Yangli Veritical Press Machines and Cincinnati Milicron Plastic Injection Machines, were not provided with Safety Data Sheets to notify employee(s) of hazards associated with the chemical.  b) Production Facility, Space Center Dr.: On or about May 20, 2014, employee(s) where exposed to chemical hazards from four unlabeled chemical containers which were stored in a flammable cabinet and did not have a Safety Data Sheet readily available to employees.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:   Production Facility, Space Center Dr.: On or about May 20, 2014, employee(s) where exposed to chemical hazards from four chemical containers which were stored in a flammable cabinet and were not clearly marked with the chemical contents or hazard specifications.   Improved Piping Products, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312856057, citation number 1, item number 3, and was affirmed as a final order on 05/04/2010, with respect to a workplace located at 4311 Director Dr., San Antonio, TX 78219.
