29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    Production Facility, Space Center Dr.: On or about February 11, 2015, and times prior there to, the employer did not implement or maintain an effective written Hazard Communication Program to train and inform employee(s) who handle, use, transport or are potentially exposed to hazards from chemicals and/or compressed gases, such as but not limited to, Oxygen, Compressed (100%),  Acetylene, Dissolved (100%), and AW 46 6K Premium Hydraulic Oil (Hydrotreated paraffinic distallates 95-100%, Zinc salts of dithiophophoric acid < 0.5%).

29 CFR 1910.1200(g)(8): The employer did not ensure that material safety data sheets were readily accessible to the employees in their work area during each work shift:    Production Facility, Space Center Dr.: On or about February 11, 2015, employee(s) who stored, handled, and utilized compressed gas cylinders containing Oxygen (Compressed, 100%) and Acetylene (Dissolved, 100%) for welding tasks, were not provided with Safety Data Sheets to notify employee(s) of hazards associated with the compressed gases.      Improved Piping Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 977766, citation number 1, item number 2 and was affirmed as a final order on 09/24/2014, with respect to a workplace located at 4311 Director Dr., San Antonio, TX 78219.
