One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc: Compression # 2 Area, 5040 Space Center: On or about February 11, 2015, and at times prior thereto, employees were exposed to a caught in hazard, while using an Optimus Lift2k Lift Table to lift compression tools that did not have a guard to protect the employees from getting their foot caught between the table and the floor.

Abrasive wheels were not used only on machine provided with safety guards as defined in paragraph (c)(1) through (4) of this section: a) Production Area, 4311 Director Drive: On or about February 11, 2015, and at times prior thereto, employees were exposed to the hazard of being struck by flying projectiles, while using a Makita Angle Grinder without a guard to grind out defects on the pipes. b) Compression #2 Area, 5040 Space Center: On or about February 11, 2015, and at times prior thereto, employees were exposed to the hazard of being struck by flying projectiles, while using a DeWalt Angle Grinder without a guard to grind off bolts on pieces of equipment.
